Abstract
Congenital diaphragmatic hernia is one of the surgical emergencies, especially in the newborn and
infant, because most of the anomalie accompany the herniation of the intra-abdominal viscera into the
pleural cavity resulting in cardio-pulmonary embarrasment. According to review of foreign literature,
the most common site of postero-lateral diaphragmatic hernia is the left side and very rare on right.
There were under 100 cases reported to Rt. postero-lateral hernia in world, but there were no cases
reported in Korea during the past 10 years.
The purpose of this paper is to present the author¢¥s recent experience with a case of the congenital
Rt. postero-lateral diaphragmatic hernia which was treated surgically in the Dept. of Surgery, Ewha
Womans University College of Medicine.
The case was a 4 months male infant admitted with symptoms of vomiting, dyspnea and high fever
on Aug. 30, 1971. Operative findings shows that almost the entir small bowel herniated without sac
through the postero-lateral defect of the Rt. diaphragm. These were replaced and opening was repaired
with 4-silk interrupted mattress sutures through trans-abdominal approach. The postoperative course was
uneventful except for a mild aspiration pneumonitis and he was discharged on the 13th postoperative day.
Our study and surgical experience was discussed and the literature reviewed.
